| Athletics Valley View School Watchung, New Jersey All students participating in athletics MUST have an up-to-date Sport Physical on file in the nurse's office. Dr. Frank is no longer giving physical exams to our students. It is best to have the physical done by the child's physician. If you're in a bind and unable to schedule one with your physician, Complete Care on second street in Scotch Plains does them. The sport physical is good for 365 days from when it was given. The official New Jersey Department of Education Form that MUST be completed by the Physician and submitted to our school is listed below.
