Nomination Form - 2017

Administrators, teachers, staff members, parents, or community members may nominate teachers. NO STUDENT NOMINATIONS WILL BE ACCEPTED.  Please see below for additional information.

You may nominate a teacher electronically by completing this form or you may drop off a completed form at each school office or the Board of Education office. Click here to print a form.  Nomination Forms must be submitted by 12:00 PM, May 19, 2017.

The nominee should be a member of the tenured staff. All names are listed in the drop down field. 

Please tell why you are nominating this individual for the 2017 Teacher of the Year Award.  The following criteria can be used as guidelines for your nomination: 

  • Has high expectations for all students, challenges students to think critically and creatively, differentiates instruction to meet the needs of diverse learners, makes learning authentic and relevant, creates opportunities for inquiry and real-world problem solving, and integrates the use of technology
  • Establishes a culture for learning in the classroom and has an excellent rapport with students
  • Inspires a love of learning in students, nurtures students’ social and emotional intelligences, and encourages kindness and acceptance in others
  • Contributes to the school outside the classroom, serves as a positive role model for students and colleagues, and strengthens a culture that is committed to learning and growing 

The following teachers received the award in either 2015 or 2016 and are not eligible for nomination this year:  Krystle Cianci, Cynthia Deegan, Susan deVillers, and Christopher Mehl.
