Child Study Team Evaluations

Teachers, parents/guardians, counselors, and/or administrators may refer a student to the Child Study Team for evaluation.  

Parents/guardians may request consideration for evaluation of their child by writing a request to the Director of Special Services. Please include the following information: child’s name, child's date of birth, gender of child, parent(s) full names, parent and child address, telephone numbers (home, work, and cell) and the best time to contact you, languages spoken at home, and a description of your concerns about your child.
